The form below must be signed and submitted before any and all passengers can participate in a tether balloon ride.

I understand and acknowledge that my participation in a hot air balloon flight activity as a passenger, event volunteer, and /or crew member is potentially hazardous. I understand and acknowledge that during my participation in a hot air balloon flight activity I will be exposed to above-normal risk of injury. I understand and acknowledge that although precautions have been taken to provide safe equipment and preparations for each flight, it is impossible to guarantee absolute safety.

I attest and verify that I, or my child, have no medical conditions, including but not limited to, Covid, recent surgery, back problems, medication or any other medical, physical or psychological problems which would prohibit me or my child from participating in this balloon flight activity and the potential for a high wind landing. I attest and verify that I am not pregnant. I attest and verify that I have full knowledge of the inherent risks involved with this event and agree to comply with the instructions and directions of the pilot and/or crew chief.

For and in consideration of my or my child’s participation in a hot air balloon flight activity, I for myself, my executors, administrators and assignees, and for my child, do hereby release and discharge: Lighter Than Air America, Incorporated, and CBH Homes including without limitation its domestic subsidiaries and affiliates, its officers, directors, employees, agents, volunteers, and any persons connected with or employed by each of them jointly and severally, and hold and save them harmless from and against any and all actions, claims, demands, liabilities, loss, damage, or expense of whatever kind and nature, including attorney fees, arising by reason of injury, damage, or death to persons or property connected with either myself or my child which may at any time be incurred by reason of myself or my child’s participation in/for the hot air balloon flight activity.
